Lipoprotein(a) is a genetically expressed contributor to atherosclerotic cardiovascular disease, with elevated levels present in approximately 20 percent of the population. As Lp(a)-specific therapies near availability and global guidelines now recommend at least one lifetime measurement, clinical demand for accurate and meaningful testing will continue to grow. Drs. Donato and Vasile will explore the evolving role of Lp(a) in cardiovascular risk assessment, including current testing approaches, the impact of molecular size heterogeneity on measurement, and the important distinction of Lp(a)-associated cholesterol within reported LDL-C values. The session will also highlight how these factors influence diagnosis, treatment monitoring, and cascade screening strategies for at-risk patients. A light dinner will be available for attendees.

Pathogenicity of Monoclonal Immunoglobulin Light Chains

Monoclonal light chains, produced by malignant plasma cells, play a central role in the pathophysiology of multiple myeloma. In some cases, these light chains are generated alongside intact immunoglobulins, while in others they are produced independently. Elevated levels of free monoclonal light chains are closely associated with renal impairment and a more challenging clinical course. This session will examine the clinical relevance of these findings, including established thresholds for identifying light chain–predominant disease and the distinctions between kappa and lambda chain involvement, offering valuable perspective for laboratory evaluation and patient care.
